Health System Navigator Program

This project is designed to help people from refugee backgrounds with chronic health issues access needed services and supports.

The service helps clients to:

  • Understanding their condition.
  • Making appointments.
  • Understand referral letters.
  • Organise specialist appointments.
  • Attend appointments if needed.
  • Better understand medication.
  • Provide advocacy.
  • Access prevention screening such as breast checks, pap smears, bowel and prostate screenings.

Eligibility Criteria

The Health System Navigator Program is designed to assist people from a refugee background with chronic health conditions such as heart disease, stroke, cancer, respiratory disease, diabetes, hypertension, physical and mental disabilities, and hepatitis.

Referral pathway

The following service providers can refer to this program:

  • BCHS clinics,
  • General Practitioners,
  • Paediatricians,
  • BCHS Humanitarian Settlement Program (SP) and Settlement Engagement and Transition Support (SETS), and
  • other health services.
